Mandatory Applicant Forms (Required before an admissions decision can be made)

Records Release Form
Needs to be completed and signed, by parent, for all incoming K-12th grade students, and given to their previous school. (School will then send records to CSY directly) **

Pastor's Recommendation Form
Required for all new/prospective CSY families (1 per family). To be given to Church pastor, by parent.

** If applicant has taken standardized testing within the past year, former school will send those scores. OTHERWISE, please contact Amy Hilton ( This e-mail address is being protected from spambots. You need JavaScript enabled to view it or 717-767-6842 ext. 221) to schedule CSY standardized entrance testing, for all students entering grades K-12th (required for Admissions Decision). Entrance testing fee: $35 per student

Required before 1st Day of School

PA Physicians' Physical Form
~ ALL students entering kindergarten, 6th and 11th grade
~ ALL students who are coming to Pennsylvania from out-of-state
~ ALL students who have been home schooled

Additional Forms

All incoming students must submit copies of Immunizations Record (K-12th) and Birth Certificate (PreK-12th) *prior to 1st day of school*
