Pennsylvania Physicians' Physical Form

ALL students entering kindergarten, 6th and 11th grade,
ALL students who are coming to Pennsylvania from out-of-state
ALL students who have been home schooled
must have a completed PA Physicians' Physical Form prior to beginning school.
