Sports Physicals Information

Each student athlete planning on participating in any sport at CSY must have a physical form on file in the athletic office before the first day of practice. CSY will offer physicals to our students at the beginning of the school year for a donation to CSY of $10.00. The Doctor will be here to do as many physicals as needed on the provided date.

 10-11 Comprehensive Athletic Physical Form
CSY Students MUST use this form.

Since the Fall of 2007 all CSY Student Athletes have to print off the Comprehensive physical form (above) and have it filled out by the athlete, a parent, and a licensed physician in order to participate. The benefit of this new form is that it will be good for all three sports seasons.  Please plan on making sure this form is filled out. THE FORM MUST BE PRINTED OUT AND FILLED OUT AS MUCH AS POSSIBLE before coming to school for your physical.
